The Sacrament Of Reconciliation
Our frail human nature suffers many different types of breakdowns some are physical in nature, but many more are breakdowns in our relationships with God and others. These breakdowns in our relationships, all of which involve a turning away from God, are called sin and require recognition of the fault and some process of restoring the relationship with whomever was wronged.
The Sacraments Of Christian Initiation
Christian initiation is accomplished by means of those sacraments which establish the foundations of Christian life. Believers are born anew by Baptism and strengthened by Confirmation and nourished by the Eucharist.
Catholics are usually baptised as infants and then receive Eucharist and Confirmation between the ages of about 7 and 12.
Adults who wish to become Catholic can, after a period of preparation, receive the three sacraments at the same time. If they have been previously baptised in another Christian denomination, they are not to be baptised again.
In some Eastern Catholic Churches, such as the Maronite Church, infants are fully initiated at the time of their Baptism. That is, they are baptised, receive their first Eucharist and are confirmed.
Baptism
This sacrament is called Baptism because of the central rite with which it is celebrated. To baptise means to immerse in water. The one who is baptised is immersed into the death of Christ and rises with him as a new creature . This sacrament is also called the bath of regeneration and renewal in the Holy Spirit , and enlightenment because the baptised become children of light .
Confirmation
Confirmation confirms and strengthens baptismal grace. It is called Chrismation in the Eastern Churches because the essential rite of the sacrament is anointing with the holy oil of chrism.
Eucharist
The Eucharist is the very sacrifice of the Body and Blood of the Lord Jesus which he instituted at the Last Supper:

The Sacrament of Confirmation is the second of the three sacraments of initiation because, historically, it was administered immediately after the Sacrament of Baptism. Confirmation perfects our baptism and brings us the graces of the Holy Spirit that were granted to the Apostles on Pentecost Sunday.
Names For The Sacrament
In the past, the usual name of the sacrament in official documents of the Catholic Church was Extreme Unction , a name attached to it as it was administered only to those near death. Peter Lombard is the first writer known to have used the term, which did not become the usual name in the West till towards the end of the twelfth century, and never became current in the East. The word “extreme” indicated either that it was the last of the sacramental unctions or because at that time it was normally administered only when a patient was in extremis . In the early 1970s the official name was changed to Anointing of the Sick to reflect Church teaching that the sacrament is to be conferred on those who are “dangerously ill”.
The sacrament has also been known by various other names in Western Christianity throughout the years, including: the holy oil or unction of the sick the unction or blessing of consecrated oil the unction of God the office of the unction. Anointing Of The Sick
Anointing of the Sick, formerly known as Extreme Unction, is a sacrament that is administered to give strength and comfort to the ill and to mystically unite their suffering with that of Christ during his Passion and death. This sacrament can be given to those who are afflicted with serious illness or injury, those who are awaiting surgery, the weakened elderly, or to ill children who are old enough to understand its significance. A person can receive the sacrament as many times as needed throughout their life, and a person with a chronic illness might be anointed again if the disease worsens. The rite can be performed in a home or hospital by a priest, who prays over the person and anoints their head and hands with chrism . The priest may also administer the sacrament of the Eucharist if the person has been unable to receive it and can hear a confession if so desired. If a person is at the point of death, the priest also administers a special Apostolic blessing in what is known as the Last Rites.

What in the judgment of the Catholic Church are properly described as the last rites are Viaticum, and the ritual prayers of Commendation of the Dying, and Prayers for the Dead.
The normal order of administration of these three sacraments to the dying is: first Reconciliation , then Anointing, then Viaticum.
Only a priest or bishop can administer the sacraments of Reconciliation and Anointing of the Sick, but a lay person may give a dying person Holy Communion as “Viaticum, the Last Sacrament of the Christian”.
